Records are over written

hi every one

I have problem which is as follow:

when I put new record in my dataentry form it over writes on my previous
records so can you help me solve this promble

Could you provide a little more detail about what you are doing and what
you are observing?
 
In My Data base I have A table called CustomerAcount when the first Customer
of that tabel which have one Starting balance Record make payment or
withdrawal the New Record replaces the old one
