Recording from reel to reel recorder to a CD

How can I record from a reel to reel recorder to a CD ? On the back of my reel to reel recorder I have a left and right channel out put jack. Can I purchase a mini-to-RCA "Y" cable and connect it to the microphone input on the sound card on my pc? If so what do I do from there to create a CD of the reel to reel tape?
