I'm trying to establish a medical excel file for my mother.
She has a complicated medical history and everytime I think I've created an
effective excel chart and bring it to the doctors, I find I'm missing
critical data.
Are there any existing personal medical history charts that I can draw from
to create a more complete or comprehensible history.

You may have better luck finding Excel templates in an Excel newsgroup
- this newsgroup is for Microsoft Access, not for Excel.

But I doubt very much that you'll find a template that matches your
specific requirements in any case. It sounds like you have a very
specific and personal set of requirements; it would be unusual for
someone to have created and posted exactly the same structure!

If you have Access, i.e. as part of Office Pro, you may want to
consider using Access for this purpose rather than Excel in any case;
Excel allows a lot of freedom, perhaps too much for this purpose. A
well designed database can be set up to enforce consistant design.
It's not a trivial process but we'll be glad to help if you post back.
