Reappearing "Save As" Box

We have a computer at work that, when booted, opens the "Save PDF File As"
box. In the "Save As" field it wants to save the file "My
Documents/Invoice". The box can be closed but it reappears in about two
seconds. We can't close it. Any help would be greatly appreciated

First try saving the file with a new name from that prompt. When that
doesn't work, look at this.. yours isn't a folder, but probably is a
unique entry in a Startup location:

You could do some clean boot troubleshooting to see what program is causing
this to happen.
