RD and TOC fields in tocs across multiple files

I've read all the info about using RD to create a toc from more than one
file. However, is the only way to get it to work is to manually mark the
entries in each file that I want included in the toc by using Alt+Shift+o?
Also, the links in the toc don't work, but I'm assuming that's b/c W is only
writing only text rather than a link in the toc?
 
You can use any method of producing the TOC (and Index) - the RD field just
tells Word which files to look in.

If you have used the built-in heading styles getting a TOC based on these is
very easy (see http://www.shaunakelly.com/word/toc/CreateATOC.html, which
also covers the RD field). However, there are lots of other options for
automatically generating the TOC, such as picking up other styles, or basing
the TOC on outline levels (the levels you see in the Outline View). You
might want to look at
