How can I make Start -> Programs always display the full programs list
instead of a micro version that I have to click extra to get what I'm
after?

Right click Start Menu | Properties | Customize

If you are using XP Start Menu, then :
Go to Advanced tab | Untick "Scroll Programs" from list and click OK

For Classic :
Untick "Use Personalized Menus" and "Scroll Programs"

Click OK
