Is it possible to print both the attachment icon (or
indication that an attachment is enclosed) and the full
Frame with recipient, subject line etc in the same print
job. Rich Text supports the icon and HTML supports the
frame. Is there a setting or work-around to get both...?

Outlook does not consistently print the attachment list for HTML messages. Other format messages should be OK. According to Microsoft, this can be fixed for OUtlook 2000 by applying one of the updates discussed at http://support.microsoft.com/?kbid=271562. I haven't had a chance to try these myself.

Anotehr solution is the Print on Demand utility from http://www.sperrysoftware.com/jcPrintOnDemand.asp, which adds two new buttons to print the message with the attachment names appended and to print just the attachments.
