print Outlook form on paper

I'm trying to create a form in Outlook. I have realised that when I print the
form it is only putting the fields entered in a normal email style print. How
can I get the form to print, as I would actually see the form? I.e. the Print
out must look the same as the form.
 
See http://www.outlookcode.com/d/customprint.htm for various solutions to the limitation that Outlook forms don't support WYSIWYG printing. The Word template technique is the most flexible and highly recommended.
