PPPoE - broandband using username & password - PPPoE disabled

i have direct cable connection, which requires username
and password to start. so when i try to create new network
connection in Connect to Internet > Set up mannually >
Broadband connection requiring username and password is
diabled already.. how to create this type of connection.
