Powerpoint 2003 does not respond after attempt to insert video.

After trying to insert a video on a Powerpoint slide, a black block appears
in the top-left corner of the screen and Powerpoint no longer responds. The
black box is less than a quarter of the screen size and does not cover up the
menu bar or the tool bar.
 
Sounds as if PPT is unable to extract the first frame of the video as a
place holder. What sort of video file is it and what is it's origin?
