I receive a "unable to process" message on exiting Outlook. I am using the
Personal Folder Backup Utility and it configured to back up the .pst on
exiting. Even a simple copy of the .pst file does not work, it appears that
something is keeping the file open. Safe mode it works, and I have also
disabled most of my tasks. Any input to this would be much appreciated.

After exiting Outlook, and after waiting about a minute to let the PFB utility finish (if it runs), check Task Manager's Processes tab to see if outlook.exe is still listed. If it is, and sometimes there may be more than one instance, that means it did not shutdown properly, got hung, and got left in memory. Kill those remnants (and winword.exe, too, if you have Outlook configured to use Word as your e-mail editor). It could be that you have an old instance of outlook.exe that is hung, still in memory, and still has open file handles on your .pst file(s).

Otherwise, when there are no outlook.exe processes listed in Task Manager's Processes tab, you can use the oh.exe (open handles) utility in the Windows 2000 Resource Kit (works in Windows XP, too, but then you never mentioned WHICH version of Windows that you use) or you can use the free handle.exe utility from SysInternals. This will tell you which process has an open handle on your .pst file(s).
