Outlook to Word Template

I used to have a Word Template that prompted for Month and Year, and would
download Outlook calendar information into a monthly calendar view. I got it
from Microsoft, but can not locate it again. I recently got a new laptop, and
the file did not get copied to my new PC.

HELP! I really love this tool!
