I have Outlook 2003. Is there a way to have Outlook check for reminders
on startup without having Outlook running? Basically, I am looking for
some sort of agent that runs in the background checking for any
reminders, etc. Possible?

No - Outlook must be running to work. There are third party notification
programs listed on slipstick.com that you can try.
