No matter how many times I reset it, the address book keeps returning to
sorting by first name when I restart Outlook. Is there a setting in Outlook
or the registry to correct this?

You need to post your mail support mode for us to answer.

Depending on your version and mail support mode of Outlook, you can sort the
Outlook Address Book this way (check for your mode on the second line of the
Help menu > About):

97, 98/2000 in Corporate/Workgroup mode:
Tools menu > Services > Highlight the Outlook Address Book service > Click
Properties > Set sort order

98/2000 Internet Mail Only:
Tools menu > Address Book > View menu > Sort by

I am using Internet Mode only with security update.

I regularly resort the list, but sooner or later it automatically defaults
back to sort by first name. This is annoying because when I use the address
book to add names it will bring them up in first name order. I will then
have to close out, go to the address book and sort by last name and then go
back to adding email addresses to the email.

Unfortunately, this is not an Outlook issue we can do anything about. IMO
does not have all the features of the full version of Outlook. It uses the
Windows Address Book. You can always switch to the full version if the
feature is important enough to you, but you'd have to way it with the other
pros and cons of Corp/Workgroup mode.
No current versions of Outlook have this limitation.
