Outline Numbering Long Document

I have a document that is 922 pages long. My numbering is fine until page 223.

The previous section is numbered correctly.
4.5
4.5.1
4.5.1.1
4.5.2
4.5.2.1
etc. The next sections is formated exactly the same but the numbering is wrong.
4.4
4.4.1
4.4.1.1 etc
Then the section after that goes back to 4.5 etc. I'm using the same formating on all of the sections but it doesn't seem to be working when I get further into the document.
Is there something that I can look for that might give me a hint why this is happening. I've tested a number of different suggestions that I have found and nothing seams to be working.

You have two different numbering schemes in the document - my bet is that on
some previous occasion someone has tried to 'fix' a numbering problem by
fiddling with the start at number, but you don't want or need to search for
the exact reason the numbering is wrong now to get it right.

You don't say whether the numbering is applied via heading styles. If it is,
the first thing to try is using Ctrl-Q on your numbered headings to reset to
paragraph style - this will reapply the numbering defined for each heading
style.

If you are not already using styles or the numbering is still not working
correctly then you need to set the numbering up following the instructions
