I doubt that this is a per-document variation. Rather, it depends on the
context. There's a good reason why the "shortcut" menus are also called
"context" menus. Which one you get depends on the type of text you're in.
You can get an idea of what the "contexts" are if you open Tools |
Customize, select the Toolbars tab, and check the Shortcut Menus item. This
will display a Shortcut Menus toolbar with Text, Table, and Draw categories.
If you click the down arrow beside each, you'll see how many different menus
there are (there are so many for Text along that they won't all display on
my screen). You can add Synonym to additional menus if you like; find the
instructions at
http://word.mvps.org/FAQs/Customization/AsgnCmdOrMacroToToolbar.htm. One of
the easiest ways to do this (which doesn't require finding the command in
the Customize dialog) is to find a menu that has the command and then use
Ctrl+Drag to copy it to other menus. This takes a little practice and
eye-hand coordination, however!

In some of my documents, I get the option Synonym when right-clicking a
word, which I find very useful. In other documents, this option does not
appear. How do I change the settings so that I get this option in a
document?
