Opening to bookmark

I have a set of word files stored on Sharepoint Services. Until recently, I
could open these documents at a particular bookmark by creating a hyperlink
comprised of the document URL (in Sharepoint) with #<bookmark> appended.
With the latest batch of updates to Word 2002, this appears to have stopped
working.

Can anyone suggest a way to open a document to a named bookmark?
 
I was able to change this behavior through the 'advanced' settings under
'file types', by telling Windows to open the document in the same window.
This corrected the problem for explicitly named bookmarks, but I'm not able
to use this to access a link to an internally-generated name linked to via
cross-references, such as those generated for headings in the table of
contents. Any idea what the formatting should be for externally linking to
such links? The address bar notation seems to adopt a '...\_TOC<num>'
convention, but when trying to link using that, you just get a '404' error.
