Old password problem

I can't get rid of an old password. I've changed the password in Outlook and
in Internet Explorer but from somewhere the old password keeps popping up. I
reformatted the drive and reinstalled Windows and still that old password
persists. Where is it coming from and how do I get rid of it?

This password was indeed for an old version of Outlook a couple of years ago
when I was using Windows 2000. I don't know which version of Outlook or
Outlook Express. I've formatted the drive, reinstalled XP and still when I
open the latest version of Outlook Express it brings up this window with the
dialer and the old password. If I open IE without dialing first, it'll also
bring up the same dialer box with the old password. You'd think formatting
the drive would have cleaned it out, but it persists. I've reconfigured
Outlook and IE with the new password. I've had a dozen passwords in between
and somehow this thing has continued to follow me. Is the only answer to
replace the hard drive or does the BIOS store Outlook information?
