No Hosts File

I found that on the anti-spyware sites that I've been
reading, it said that you need a hosts file in order to
keep these hijackers out of your computer.

I don't have one. I have a file folder called Hosts in
C:\Windows\Cache but there is no file in there at all.
How do I create a Hosts file? Do I just make it a .txt
document or does it have a certain extension? Also, what
does the format look like?

Your HOSTS file (may be hidden) is HOSTS.SAM
