newbie question; working with Word

Please dont flame my ignorance here, new at this specific function.
I have a document created in word with fields such as this (Name:_________),
etc.
I want to lock this page such that others can enter their respecive data
without changing the layout of the page. I have used Word for many years, for
very different purposes and do not even know where to start to learn to do
this.
any help greatly appreciated.

Hello there,

You will need to use Form fields for this.

1. Type out the document leaving gaps for the data that
the user will add.
2. View + Toolbars + Form.

The first 3 icons are for Text, Check Box and Drop-Down
fields.
The fourth icon changes the properties of the field you
choose, eg. If you choose a text field you can set the
maximum length, and/or case of text.

3.Click in the document where you want the first user
entered field.
4.Click the field icon you want (probably Text field).

This will insert a grayed out box. It will not be active
until you lock the form.

5.Continue to add the fields anywhere you would like the
user to enter information.
6.When you are finished click the last icon on the Form
toolbar to lock the document.

This will only allow users to enter info in the field you
have added. They can use Tab to move from one field to
the next.
