Cannot find a downloadable template to use in Outlook for phone messages.

You could try this one. Not sure what version of Outlook you're using but
it might work with the newer versions.
 
I'm sorry, you must have forgotten to add the link, or the instructions.
There is nothing in your reply that gives me an answer. I am using Outlook
2000.
