Just guessing here. Do you happen to have one of Creative's soundcards,
soundblaster, Audigy, X-Fi, etc?

Have you right clicked on the Speaker icon in the Notification Area of the Taskbar, selected Playback Devices, right click an open area in the dialogue box, check both Show Disabled Devices and Show Disconnected Devices and tried to configure your audio playback devices? You might have to right click on the appropriate device and enable it. Also, what does Device Manager show for for audio playback device?

Not sure if this is a permanent solution but I had a similar experience
after installing the Logitech drivers.
-sound playback became inconsistent, some time it worked, sometime it
didn't
-CONTROL PANEL -> SOUND showed multiple sound devices
-I discovered that Yahoo Messenger was enabled as a VoiP application by
Logitech
-YIM was using the wrong default speaker driver
-In YIM, main menu MESSENGER -> PREFERENCES -> 'Calling & Audio' section
-set the correct default speaker set
-double check in CONTROL PANEL -> SOUND
-reboot (old habits die hard)
I have been running through a couple of docks, undocks and it hasn't
misbehaved but again, I am not sure I found the right solution so mileage
may vary. If anyone reading the posts knows the 'real' technical answer, I
would love to hear it.
