Multiple forms in one document - Word 2003

Word 2003 - I have a single page input form that will be included in a finished document that is segregated into 7 categories - that is to say that each category has many sub categories with the individual input for each sub category being a single page. The form is standard and repeated for each page

My delimma - I cannot find a way to input the forms on multiple pages of the same document (I would like to save as a document by category to be able to make best use of index and content) without saving each input form separately. While this will be fine for the printed version of the document it becomes a cantankerous way to manage and update

Any help from those I failed to confuse?

Well, wasn't that a nice response. In as few keystrokes you could have suggested which newsgroup and which knowledgable source. My search here showed it had not been answered - so back to blind searching - thanks a ton...
