With the customize dialog displayed, right-click on the button and select
.... (drum roll)
Default style
(g)
It is not very intuitive. But what it means is show only the button on
toolbars and both the button and text on menus.

Suzanne's fix partially worked. I had taken out the description and used
spaces because the user didn't want the button to say Save As. I can get the
tool tip to work if the Save As is in there with the button that I chose.
However, he wants the icon without the text. Is there a way to get rid of
the text that is appearing on the button? There must be a way since none of
the default buttons have text. Thanks...
