Missing Header/Footer Toolbar

When a user opens the View - Header/Footer menu, the Header/Footer toolbar is missing. I have uninstalled and reinstalled Word, replaced the normal.dot template, and tried customizing the toolbar to re add but no luck. When customizing the toolbar, the Header/Footer box is unchecked and will not allow you to check it. Any ideas on how to get the toolbar back

Are you still unable to check the toolbar if you open Tools | Customize with
the insertion point in the header pane?

Yes we are unable to check the Header/Footer toolbar while the insertion point is in the header pane
BTW, this problem is in Word 97

Have you tried rebuilding the Data key as described in
http://word.mvps.org/FAQs/AppErrors/MissingMenusEtc.htm
