Menu bar, etc. disappered

Menu bar, standard buttons and address bar are all disappeared. How can
I restore these?

From where. IE,Office , Windows Explorer,etc
Have you tried to right click on an empty place on top by the Toolbars that are showing. Is Standard Buttons selected
 
It is Windows Explorer. Right click empty space on the left pane and
select Open, then everything come back but that does not stays after
closing Windows Explorer.
