Medication templates

I cannot find a log that is specifcally for tracking blood glucose readings
for diabetes. Should I modify an existing log and if so, which one.

If you've searched the web, and had no luck, then I would think not.
However, a database for this purpose would be very simple to throw together. It would
be a flat datbase of daily entries...
Just a simple "diary" table (I'm not familiar with the exact Results you expect...
these are samples only)

I'm guessing, from your post, that you're fairly new to Access. You might
want to consider a commercially available app. The "Glucose Journal and
Database" is one such, and allows you to download a demo version. It runs
less than $25 (USD) and allows you to actually order it from inside the demo
database.
