making a template

I want to be able to make a template of a document that is used it hte office. But the problem is that I want the template to be in the background and the user types in the spaces provided to fill out the form then print it out with out messing with the original document. Right now I have made coppies of this form and we hand write on it. O would like to be able to fill it all out on the computer. Is that possable. If not is their a program out their that can do that.
 
Word can easily do that but you will need to create the electronic form
using Tables and the Forms toolbar.
