Making a questionnaire in Outlook

Is it possible to create a questionnaire in the body of an e-mail within
Outlook? I'd like to have the receipients of the e-mail be able to check
boxes and use radio dials to answer "Confident, Somewhat Confident, Mildly
Unsure, Unsure, etc...." is there a way to do all this within Outlook?
