mail merge toolbar isn't active

We have a few saved documents that at one time were mail merged but now the
fields show but the toolbar is inactive. What do we need to do?

Most of the buttons on the mail merge toolbar are inactive until a data file
is attached to the displayed merge document. Documents that are created as
the result of a merge are simply documents and not merge documents.
