Looking for Template for Homework Assignments for sick student

Does anyone know of a good template for a document that I can send around
with a student for a sick student? It would list subjects and assignments for
the day.

You don't need a template. Just type the information, you know what needs
to be there and no one else does. Then save it as a template yourself, with
the info that will repeat.
