junk e-mail not detected

Do I need to do something special to turn on the junk mail
option.
I have it set to "high" and have added several domains to
the block sender section. None of the e-mails I receive
from these domains are block.
 
What's your mail setup? Exchange? Cached Exchange? IMAP?
