Items in the OPEN WITH menu

I am trying to add MS Word to the OPEN WITH menu. I have browsed to the Winword.exe but when I click OK nothing happens. I am trying to open a .TXT file with MS Word

Also how do I remove items from the OPEN WITH menu?
