Is there a simple way to type texy on a line in Word?

Hello Everyone- I am trying to make a fairly simple document that a person
could open up and type on the lines that I have already put there by using
shift and the line tab: Name___________________

But it looks like this: Name______Kim_________

It won't be used a lot so I don't want super fancy form but maybe that I the
only way (or go buy an old-fashioned typewriter!)

I have tried the OVE button, insert, and simple stuff but nothing works! I
thought I was adept at Word but I now feel DIM! It's taken way too much
time and I am hoping someone here can please, please, please tell me if
there is a simple way to do that? I appreciate any information very much.
