In Defrag Report, where is " Excess folder fragments " defined ?

My defrag report contains the count, in my case 185,
of items labeled " Excess folder fragments ".

Neither the Help file, nor the Resource Kit, nor the Microsoft web site Search
gives any clue as to where " Excess folder fragments " is defined.

I can understand what folder fragments are,
but
what makes folder fragments " excess " ?

Also, how do I identify which folders are fragmented ?
