where do I find the icon for "protect form" seen in microsoft word?

Display the Forms toolbar, do a screen capture, crop, save.

Not necessary. Open Tools | Customize, display the Forms toolbar,
right-click on the Protect Form button, and choose Copy Button Image. Close
the Customize dialog and paste the image anywhere you like.
