I need help with forms

Here is the Problem.
We use a Message form that was created under public folders.
Some people open it up and it looks right.
Person X will send the message to another user. The other user gets the
message but it opens as a regular email. No check boxes nothing. Plus the
Icon in their in box looks like a e-mail Icon and not a message icon.
 
1) It is Published under all public folder in a sub folder named Forms.
2) Unchecked
3) I don't think the form has any code. I click on Form then view code and
it is blank
4) Outlook 2000 and outlook 2003

Because the form is published in a folder, it will work consistently only on
items stored in that folder. If you want the published form to be invoked
for messages in the Inbox, you must publish the form to the Organizational
Forms library or each user's Personal Forms library.

Alternatively, you might checking the "send form definition with item" box.
That will embed the form definition in the message, at a cost of increasing
the size of each item.

more problems.
When I get the message and click forward I get this error "you do not have
permission to send the message on behalf of the specified user". I
understand that it is somehow keeping the From: account. This does not
happen to everyone. I do believe the form was made with office XP if that
helps. I looked at Article ID : 323928. Was no help.

I published a form and I need to delete it. How do I deleted a published form?

More Problems.

I get "you do not have permission to send the message on behalf of the
specified user" when the message is forwarded. Of course it fails to forward.
I understand that it is keeping the FROM: account and not replaceing it with
the current users account. I looked at Article ID : 323928 it was no help.
