Well... a couple of concerns.

This is a volunteer newsgroup. We're happy to answer specific
questions free of charge; but to completely design and set up an
entire database is rather beyond what would be reasonable. Are you
having specific problems? Do you have some ideas about the data model
that you'ld like to discuss?

Another is: if I were one of your clients, I'd be more than a bit
leery of having notes on my therapy stored in Access. Access databases
are NOT highly secure, even with Access workgroup security
implemented. If you're located in the United States, I'd expect such
databases to come under the very stringent HIPAA patient-privacy
restrictions. Are you certain this is the best tool?

Basically what I was looking for was a template, I wasn't looking to have the
system written for me, I thought that someone out there may have written
something similar at one time or another that I may have been able to adapt
to my requirements rather than sitting down and writing the whole thing from
scratch..

I am an Australian registered psychologist, the regulations for record
storage are very stringent, the actual database would be as secure as my
current paper based records, I planned on storing this to cd/dvd-rom, not on
a mainframe. I need something that allows me to report on time spent etc.

hey you should just find someone who specializes in access to get this
started for you. i would reccomend a microsoft certified solutions
provider.. i know of one in seattle-- email Ken at (e-mail address removed) for a
quote; tell him you need to do it on the cheap and fast and you just
want to start with something that works

hope that's not a bad thing to reccomend (as they postings are supposed
to be non-commercial) ; i just know that he can do small, cheap apps
like this.

tell him you want a $5,000 solution and see what he says

i think that $5k to do someting like this is reasonable; it could save
you a lot of time and heartache

There's no business model that makes it worthwhile for anyone but Microsoft
to create Access templates; templates are for letting many users do some
(usually minor) choice and customization of a basic database application.

Thus, you are far more likely to find "a sample" or "an example" database
for a particular application than to find a "template" similar to the
templates that come with the Access product.

I have not seen a sample/example database for a psychology practice, but
many "service industry" sample/examples might serve as a good starting
point -- they would have customer information, appointment information,
records of service, etc.. With a little "out of the box" thinking, something
like an "automotive service shop" database could serve as your "jumping off
point".

The FAQ site, http://www.mvps.org/access has a lot of links to sites with
Access information, some of which include example databases. Your best,
perhaps only, chance to find a template would be to start at
http://office.microsoft.com and search for one that might apply. Another
site with links to Access resources is http://ntaccess.tripod.org.
