Hyperlink - Database Postcodes linked to online mapping prog

Any ideas how I link a field in my database to an on-line mapping facility?

For instance:-

My post code is XX23 0ZZ, I would like to click a link that will take me to
say.. multimap and allow that address to be automatically displayed.

Any ideas?
