How to use template MS Excel 5.0 Dialog in Excel 2002

When you right click on a tab e.g. Sheet 1 and select Insert a window comes
up with a few options - one being MS Excel 5.0 Dialog. I am looking for a
tutorial ot idiot's guide to using that template and its tools
 
Excel 5 Dialogs are provided only for backward compatibility, and have been
replaced by UserForms. I would hesitate to use the former as MS may
discontinue support for them at some point.
