How to make an OMR sheet?

Can someone tell me how to make an OMR sheet? PLz tell what program to
use,& also the steps to proceed.
(i can't understand how to paste something, say a symbol, * ,
many times over in a row/column)

Hi - I don't know what an OMR sheet is.

As if i know a lot,an OMR sheet is "Optical Mark Reader" sheet.It is
used widely, the most common being asking ?'s & getting answers in
multiple choice(MCQ). I need to make them on my own.
