How to create this word document?

Hello all

I find some interesting word documents located on ISU websit

http://www.admissions.iastate.edu/app/doc/intl_gr_app.do

We can input the text input corresponding text field which is marked as shadowed area

Does anyone know how to create these kind of word document

What you are talking about - looking for - is probably what Word calls an
"online form." Check this in help. For more about online forms, follow the
