how to create questionaire in word?

I am trying to create a questionaire that will be used often. I am trying to
avoid accidently changing the original when I am entering information into
the fields for each question. How can I create a document like this?
 
1) Use a template. http://addbalance.com/usersguide/templates.htm

2) Look into what Word calls an "online form." Check this in help. For more
