how to create a pull down menu in a word doc

I would like to create a simple pull-down box in Word.

I am making a questionaire. 15 questions, and after each question a pull
down box where they can select the number that corresponds to their rating of
the idea. The pull down box would have numbers from 1 to 5.

I've poked around here and in the Word help, but am not having any luck.

Could someone show me the right path?

Hi Brian,

You could use DropDown formfields, which you give the desired numbers as choices. DropDown formfields are available on the Forms
toolbar (which you may have to activate, via View|Toolbars). For this process to work, your document will also need to be protected
(via Tools|Protect Document| > Forms).
