How do you insert text with page number?

i'm trying to insert my last name before the page number, in the upper right
hand corner of the page. I can get it to insert the page number but don't
know how to also have my last name inserted before the page number, on every
page. can anyone help?
 
View Headers/Footers and type the text in. Also notice the icons on the
toolbar that let you enter page numbers and other useful stuff.
