How do you format text and illustrations for publication?

In writing a book to submit for publication what is the best way to set out
text, figures, illustrations and index?
 
If it's a manuscript, the publisher will likely want the illustrations
separately (possibly even in separate files), with callouts in the text. If
you already have a publisher, the publisher should have supplied guidelines
for formatting. If you are submitting a manuscript on spec, get the
publisher's "Writer's Guidelines."
