How do you create checkboxes for Microsoft Word 2007?

Does anyone out there know how to bring up Visual Basic or just the toolbar
that shows the checkboxes, drop down lists, etc, - what they have in Windows
2000?
 
First go to Office button > Word Options > Popular and check the box to show
the Developer tab on the ribbon.

Click that tab. In the Controls group, click the Legacy Tools button (fourth
button in the second row). The form fields for protected forms are on the
dropdown, as well as the ActiveX controls that were on the Control Toolbox
in previous versions.

The Visual Basic editor's button is at the left end of the Developer tab,
and its shortcut is still Alt+F11.

At Office Button | Word Options | Popular, check the box for "Show Developer
tab in the Ribbon." Then click on the Developer tab, and, in the Controls
group, you'll find the Legacy Controls, which include the legacy form
fields.
