You can put them in a rectangle that looks like an option group if you want,
though it might be better not to, from the point of view of user-interface
consistency.
There is not always or necessarily a direct, one-to-one relationship between
controls on a form and fields in a table. Without any knowledge of your
database schema, or of the real-world problem you are attempting to resolve,
I can not say what fields you may or may not need.
