How do I set up a one page calendar that displays all 12 months?

I'm using Microsoft Word/Works Suite 2003. I would like to create a
calendar, however, I do not want only one month (i.e.; Jan.Feb.Mar) on one
per page. I would like to have all 12 months all on one page. Included in
this task, I would like to be able to specify My start and ending months,
i.e.; November 2004-November 2005, rather than the standard fiscal 1/01-12/31.
 
The following macro will create a "planner" type calendar. To get one that
does not start with January 1, you could create one for the first year and
one for the second year and join the two tables together and then delete the
rows corresponding to the unwanted months. (Or if you are a masochist, you
can try and modify the code)
