How do I set up a document template that automatically prompts fo.

I am trying to set up a document template that will automatically prompt the
user to enter specific information. For example, when the user opens the
template they will see a box appear asking for a "Proposal No.?". The user
can enter this information and hit enter and will then get another box that
says "Client Name?", etc. I know that I have done this before, but I don't
remember how I did it.

Use any of the four methods listed at
http://word.mvps.org/faqs/customization/FillinTheBlanks.htm. I think
the one you used before was #3.
